Is Erectile Dysfunction common?

Even though Erectile Dysfunction is a common medical condition, a majority of men suffering from this condition would never disclose this openly because of the shame surrounding it. Studies have estimated that roughly, over 18 million men in the United States suffer from Erectile Dysfunction. Though it can affect anyone at any age, it is prevalent in older men over 40 years old. How can it be managed?

The most commonly adopted method of managing Erectile Dysfunction is by using oral pills. These include Cialis and Viagra, but due to the difference in the physiological makeup and characteristics of human beings, most patients do not respond to treatment using these oral pills and others develop adverse side effects after administering these pills. In these cases, only a custom compounded solution can be effective for the treatment of this condition.
Injectable medications that are available for the treatment of this condition include Phentolamine, Papaverine, Alprostadil( Prostaglandin E1), Atropine, Chlorpromazine, Forskolin and Vasoactive Intestinal polypeptide (VIP). Oral Medications for Erectile Dysfunction

Viagra which is also known as Sildenafil can be prepared into an oral tablet which can range from 25mg to 50mg composition. When starting your dose, it is recommended that you take your pill about one hour before your foreplay or sexual intercourse. There are no reported reports of negative and adverse effects from viagra so it is perfectly safe and effective for the treatment of Erectile Dysfunction. The only problem is that it is completely ineffective in approximately 27% to 35% of patients who have tried using it and a smaller percentage have complained of some effects like headaches, dysplasia, and flushing. It has also been proven to react adversely with nitrates and Cytochrome P450 enzyme inhibitors.
Cialis which is known as Tadalafil can be prepared as an oral tablet from 5mg to a 20mg composition. The recommended starting dosage for a majority of patients is usually set at 10mg which should be taken only once in a day as per instructions from your physician. This dosage can be increased to 20mg or decreased to 5mg but this should depend on its effectiveness in an individual and his level of tolerance to this medication. Cialis is effective in the body for about 36 hours which earned it the name ”the weekend pill” just like viagra, Cialis is recommended as a needed pill which means that it should only be taken when its effects are desired. Cialis is the only oral medication out of all the others that is offered as a ”once in a day” treatment.

Injectable Erectile Dysfunction Medications

Papaverine

Papaverine is a vasodilator and its main function is to relax the body’s smooth muscles. Its pharmacologic action involves the nonspecific vasodilation of the arteries and the blood capillaries. This substantially increases the flow of blood into the penile which alternatively effects an erection. Major side effects from papaverine can include Corporal fibrosis and Priapism. These can be greatly reduced or completely avoided when Papaverine is used in very low dosages or combined with Alprostadil or Phentolamine solutions.

Alprostadil

Alprostadil, commonly known as Prostaglandin E1 has a mode of action by binding with PGE1 receptors which produce a resultant relaxation response in the body’s smooth muscles. Very little and inconclusive research has Quad mix on the general pharmacokinetics of PGE1 but it, that after its reactions, up to 80 % of it can be metabolized by one pass through the lungs. Because of its fast degradation, it lacks any notable cardiovascular effects whenever administered intracavernosally. Alprostadil can also be metabolized rapidly in the penis so when it is administered to the penis, its reactions only affect the penis and it is not transported to other parts of the body. This further assures its safety and efficacy. Alprostadil can also be used in combination with other agents, the most common being Papaverine. This combination has proven to be one of the most powerful treatments for Erectile Dysfunction. Alprostadil can also be compounded as an intraurethral gel, an intraurethral pellet with penetration enhancers and the intracavernosal injection. A large quantity of research data has proved that the injection preparations are the most effective in the treatment of E.D.

Phentolamine

Phentolamine provokes a specific reflex action by releasing norepinephrine which decreases the sympathetic flow of blood out from the veins in a specific body part or system. When phentolamine is used in the treatment of Erectile dysfunction, it is commonly administered in a combination with other agents which further enhance its effectiveness. The combinations of Phentolamine with other agents more specifically Papaverine has been studied extensively and has proven to be very effective in effecting an erection that can sufficiently last throughout a complete sexual intercourse.

Chlorpromazine

Chlorpromazine is an aliphatic phenothiazine that is indicated for the treatment of a variety of conditions ranging from nausea anxiety, schizophrenia, and hiccups. Chlorpromazine was discovered back in 1951 and it was quickly adopted by the mental health society because it has very powerful effects on numerous neurotransmitter channels. It was considered the greatest advancement in psychiatric health in the 20th century. An alpha1 adrenoceptor blocking agent which forms one of the elements of Chlorpromazine’s psychiatric profile is what made this medication a natural subject in the investigation of its efficiency in the management of Erectile Dysfunction, surprisingly substantial data has proved that it is very effective in managing Erectile Dysfunction and reducing the side effects from Phentolamine. You can also combine it with other treatment agents to form even more powerful medications.

Atropine

Atropine is a nonselective muscarine agonist which performs its functions by competing for and binding to Muscarine receptors. It belongs to a class of acetylcholine binding receptors which are involved in the contraction and relaxation of the smooth muscles and epithelial tissues throughout the body. When these receptors are activated locally in the penis by acetylcholine molecules, they lead to the contraction of the Corpus cavernosum by inhibiting the absorption of acetylcholine. Atropine, therefore, relaxes blood vessels in the penis to facilitate an erection. Nevertheless, the effectiveness of Atropine when used individually is very insignificant. When it is used in combination with other injectable treatments, it does not improve the success rates of these treatments as much as they do without it but it can substantially help to reduce the pain caused by PGE1.

Forskolin

Forskolin is an adenylate cyclase activator that acts by directly stimulating the enzymes whose function is to start and maintain an erection. Adenylate Cyclase activators trigger an increase in cyclic adenosine monophosphate which results in the relaxation of the smooth muscles of the penis. This action causes penile veno-occlusion and increases the quality of an induced erection. Forskolin has proved to be very safe and effective especially in treating patients with vasculogenic impotence and those resistant to the standard Phentolamine, Prostaglandin and Papaverine combination treatment. Forskolin acts together with Prostaglandin to increase the rigidity and the duration of an erection with little to no side effects.

Vasoactive Intestinal Polypeptide

Vasoactive intestinal polypeptide (VIP) is a very powerful smooth muscle relaxant and a vasodilator. It has an inhibitory effect on the cavernosal vessels and the human corpus cavernosum tissues. VIP is the main neurotransmitter which is involved in the erection of the penis. It is presumed that the exhaustion of this powerful vasodilator polypeptide can play a major role in the development of penile impotence. A recent study of 171 patients in a controlled research showed a response 75% out of all the patients, which indicates that this medication is very safe and efficient in the treatment of erectile dysfunction primarily in non-psychogenic etiology.
